Most services are billed on an hourly basis according to the level of service provided, needed and/or requested. The billing includes any time spent on the client’s behalf, including on-site visits, phone time with service providers and interested parties, correspondence, report and document preparation, and travel time. Expenses are included on the invoices. Unscheduled after hour consultations and visits requested have a surcharge of $125 per hour.

Life Care Manager (CM) $100 per hour
Senior Life Care Manager (SCM) $115 per hour
The Life Care Manager services are provided by Life Care Managers and Senior Life Care Managers, who are licensed social workers or nurses.  The Life Care Managers are utilized to supervise all cases to insure quality of care. The Senior Life Care Managers have advanced certification and extensive experience.  They are primary on the most complex cases.

• Negotiate with third-party payors for benefits
• Court Requested Assessments
• Complete coordination of health care

• Reviewing medical treatment options
• Referrals to appropriate health care services
• Mini Mental examinations
• Care Plan Development
• Negotiate rates and terms with health care providers
• Filing for disability and/or Public Aid (complicated or denied)

• Bill paying service
• Money management
• Providing assistance with client needs under the supervision of a Life Care Manager

• Assists in filing Public Aid applications
• Assists with Insurance claim filing
• Assists in filing for disability and prior approval

Advocacy Services:  Rate determined on case-by-case basis

• Negotiating complex medical provider bills
• Negotiating insurance issues
• Serves as an expert witness in contested guardianships
• Expert witness - rate is determined by complexity of case and the expertise of the witness

Expert Life Care Manager (ECM) $150 to $250 per hour depending on expertise.
The Expert Life Care Manager is utilized in special and complicated situations. With vast experience, advanced degrees, and special certifications, the ECM functions as an expert witness in contested court cases and oversees the most complex cases.

• Oversees complicated cases with complex medical issues and negotiations per special request
• Oversees cases that have extensive social and psychological conflicts per special request
• Crisis and conflict management is cases that have utilized multiple levels of service without successful outcomes

Additional Services Available.

• Unscheduled after hour consultations and emergency visits requested by the client or responsible party are charged a surcharge of time and a half.
• We reserve the right to charge $5 per call for calls made to our after hour answering service.
• Pre-planning for Power of Attorneys and or Executor of Estate.  Set-up fee and first year $250.
• Annual renewal for Power of Attorney and or Executor of Estate. $125
• Pre-planning for Mental Health Declaration. Set up fee $250
• Provider Care Journal  $600
• Annual renewal of Provider Care Journal  $300
