Senior Care Manager: The Senior Care Manager’s role is multi-faceted, and emphasizes both case management and the supervision of all clinical personnel involved in a given case. The Senior Care Manager essentially bears responsibility for the progress of a case, the welfare of the client and the effectiveness of other professionals assigned to the case. Senior Care Managers frequently manage 15 or more cases, some of which are highly complicated and involve health care, home management, financial issues and other challenges. Most Senior Care Managers are promoted from the ranks of Life Care Innovations’ Care Management staff. All Senior Care Managers are nurses or licensed social workers, and all have advanced education (Masters level or above). Several years of field and supervisory experience are required for this job, as is membership in professional organizations.

Care Manager: The Care Manager’s role encompasses advocacy, client support, knowledge of health care issues and, often, expertise in a particular sphere – dementia, psychiatric disorders, forensics, elder abuse, etc. Care Managers are Masters prepared registered nurses or licensed social workers, and have ample field experience that includes clinical assessments, and use of both community resources and medical referral sources. Care Managers report to Senior Care Managers and maintain substantial case loads. They are responsible for moving cases forward and appropriate delegation in order to preserve client resources to the greatest degree possible. Many of Life Care Innovations’ Care Managers began their careers at the company as Assistant Care Managers.

Assistant Care Manager: The Assistant Care Manager, also a Masters-level registered nurse or licensed social worker, often maintains a high profile in the client’s life, making frequent visits, accompanying clients to physician appointments, as appropriate, and providing other services that do not require the expertise of a Care Manager or Senior Care Manager. Assistant Care Managers take direction from both Care Managers and Senior Care Managers, and may work the case loads of more than one Care Manager. With adequate field experience and consistently competent performance, Assistant Care Managers are often promoted to the Care Manager role.

Care Associate: The Care Associate supports client needs in a more personal and non-clinical fashion. Clients frequently need assistance with shopping, driving to physician appointments, going on outings in the community or simply enjoying a restaurant meal that they would otherwise find unmanageable. Care Associates are characteristically warm, friendly and sensitive to the needs of LCI’s elderly and psychiatric clientele. As primary contacts with clients, they often note the early signs of emerging problems and promptly report these to clinical professionals on the case.
