What is “Life Care Management”?
Life Care Managers assist clients to move through the health care maze. Today the system is too complex for the normal, healthy adult. People’s abilities are compromised when they face medical, psychiatric, or social challenges. In short- they need an advocate, a professional advisor to assist them to maximize their health. Life Care Managers also assist with managing life issues. If one’s having difficulty managing their health it is likely they have difficulty managing their house and their bills.
Why hire an advocate?
An advocate can break the system down into smaller pieces. There are over 700 home care agencies in Chicago and over 400 nursing homes. How do you choose? When the system is not working for you, which it frequently doesn't, an advocate can move things forward – make things happen – help you on your road to health.

When the individual can't remember what the doctor said and so is unable to follow the advice. |

When the individual "mixes up" their medications, such as running out of medications, filling prescriptions at multiple pharmacies, continues to take discontinued medications, etc. |

When the individual calls you several times a day going over the same issues not being able to remember or execute your instructions. |

When the individual "forgets" to pay their bills and the phone and utilities get turned off. |
